The Life & Annuity Insurance Carrier Forms Pool Subscription provides insurance companies with standard forms that meet all regulatory requirements for the U.S. life and annuity markets. Program participants receive a license to use ACORD Forms and related tools.

Pricing & Eligibility

This program is open to insurance carrier companies only. The per term fee is calculated based on the net written premiums of your organization,and calculation rate for the ACORD L&A Forms category.
